The Practice Group have been at the forefront of Consultant Led Community Ophthalmology Services since 2006. We now provide services to over twenty CCG’s across the country. It is our aim to make life as simple as possible for referrers. We offer a well-rounded service, with a team of specialist triagers, a referrer helpline, a Referral Management Centre (RMC) and the facility to receive advice from an ophthalmologist on the same day. We accept referrals in a variety of formats – please see the link below to find out the best route for your area. We can treat a wide range of conditions and once received, every referral is clinically triaged by an ophthalmologist and signposted to the most appropriate service for the needs of the patient.

Mr Simon Horgan MBBS FRCS FRCOphth

Consultant Clinical Lead for Ophthalmology

Mr Horgan is our Consultant Clinical Lead for Ophthalmology. Mr Horgan completed his training at Moorefields Eye Hospital and works as a consultant ophthalmologist at St Georges Hospital in London where he specialises in retinal disorders. He has vast experience in setting up community ophthalmology services, including cataract surgery and primary care clinics run in general practice. Mr Horgan’s clinical interests include age related macular degeneration and diabetic retinopathy. At The Practice Group we understand the importance of becoming active members of each healthcare economy that we work in. We are committed to working with local GPs and Optometrists to enhance the ophthalmic care patients receive. To do this, we ran refer education sessions within our Community Ophthalmology Services to provide training to GPs and Optometrists in our service pathways, common eye care problems and red flag symptoms.

Mr Nick Jacobs MB ChB FRCOphth

Consultant Lead for Glaucoma

Mr Jacobs is our Specialist Consultant Clinical Lead for Glaucoma. He is a highly experienced ophthalmic surgeon having specialised in high volume cataract surgery. In addition to cataracts he majors in the management of glaucoma, within which his experience is invaluable in ensuring our compliance to the 2009 NICE glaucoma guidelines. Mr Jacobs uses his extensive experience and expertise of glaucoma management to further improve the service we offer to patients and commissioners, as well as develop new approaches to glaucoma management such as the virtual glaucoma clinic. We welcome your feedback here at The Practice Group so we are able to continuously improve the services we deliver.
