The Practice Group launches Complex Care Division

One of the largest primary care multi-site GP surgery operators shakes up complex care market with launch of new division

One of the largest nationwide NHS providers of primary and community based care, The Practice Group (TPG), has launched a new Complex Care division designed to deliver the highest quality home-based complex healthcare services to people with significant health conditions, long-term illnesses or disabilities.

TPG Complex Care works with doctors, nurses, independent case managers and other key advisers to ensure the care provided to individuals is tailored to a very specific set of requirements. In addition, it works extensively with clinical commissioning groups (CCGs), local authorities and the wider NHS to offer a joined-up service that caters for individuals with complex healthcare needs at home rather than in a hospital setting.

Sharon Lane, Head of TPG Complex Care, explains: “Our focus for this new division is based on enabling people to make their own choices and to be able to live their life comfortably at home independently. To do this, we listen to the needs of our patients, and in combination with their clinical requirements, create the best possible care plan. Once this has been agreed with all of the key parties, we then put the care plan in to action through the provision of a range of tailored services, from live-in and hourly care to specialist requirements such as those usually provided by nurses.”

TPG Complex Care can cater for a wide variety of specialist healthcare conditions, including acquired brain injuries, spinal injuries, learning disabilities, mental health conditions, neurological disorders, dementia and those with ventilator dependencies to name a few.
